If your child has just been offered admission into the University of Lagos, kindly take note of the following screening documents required for the admission process 👇

 

1. Screening Form (Coloured) — Sign & date pages 2 & 3

2. UNILAG Online Application Form (for Direct Entry — Coloured)

3. JAMB Admission Letter (Institution Copy — Coloured)

4. UTME Result Slip (Coloured — not for Direct Entry)

5. JUPEB / A-Level / OND / HND / B.Sc. Result (for Direct Entry — photocopy)

6. O’Level Certificate (WAEC / NECO / GCE / Cambridge — photocopy)

    * Statement of result from school is NOT acceptable

7. Letter of Reference (Original) from a Clergyman, Imam, or Lawyer

 

 

• Must have a valid NBA seal (NOT red)

• Addressed to The Registrar, University of Lagos

• Scanned copy NOT acceptable

 

8. Birth Certificate (photocopy)

9. Testimonial from Secondary School (photocopy)

🔟 State of Origin Certificate (photocopy) — must match your JAMB record
